Community-Based Programming

Excelsior’s in-home and community-based programs are designed to preserve families by helping them develop the capacity to successfully address challenges at home. By providing resources and strategies aimed at strengthening the family, we are able to preserve family unity and support at-risk youth before more intensive interventions are needed.

Our team specializes in working with youth (ages 0-24) who are high-risk for out-of-home placement, focusing on enhancing safety, improving relationships between family members, and creating sustainable community support.

Understanding that no two individuals are alike, Excelsior customizes its care to meet the unique needs of each child and their family. Every Excelsior family has access to all components of our Community-Based System of Care to ensure they receive the right service at the right time.

Programs & Services

Integrated Family Preservation Program

The Family Preservation program is a 90-180 day program that focuses on building successful families and maintaining youth in their homes. This program can be a continuation from the Family Stabilization program or may be applied to youth and families who are at risk, but not in crisis. This program can be applied to all types of families including biological, kinship, placement and/or foster families.

Integrated Family Stabilization Program

The Family Stabilization program provides up to 90 days of intensive therapy services for youth and families assessed at a high level of acuity. This program is limited to youth at imminent risk of out-of-home placement or those transitioning home from a higher level of care.

Early Childhood Mental Health & Parenting Support Program

The mission of Excelsior’s Early Childhood Mental Health and Parent Support Program is to support and strengthen families and to prevent child abuse, neglect and family violence. Excelsior’s team of highly trained licensed and Master’s level therapists provide a unique blend of mental health counseling and parenting support/education to parents and children between the ages of 0-6 in their homes and communities.

Wraparound Care

Wraparound is a team-based process where multiple systems come together with the youth and family to create a highly individualized plan to address complex emotional needs. The goals of Wraparound are to meet the needs prioritized by the youth and family, improve their ability to manage their own services and supports, develop or strengthen natural supports, and integrate all systems and supports into one streamlined plan.
